2. Service Provision and Professional Standards

Healthcare Services

  • All services are provided by registered healthcare professionals maintaining current AHPRA registration

  • Services are delivered within each practitioner's scope of practice and professional competency

  • All practitioners maintain current professional indemnity insurance

  • Services are subject to our Clinical Policies and professional standards

Service Availability

  • Services are provided during business hours and by appointment

  • We reserve the right to modify, suspend, or discontinue services with reasonable notice

  • Emergency services are not provided - in medical emergencies, contact 000 immediately

3. Client Responsibilities and Conduct

Appointment Attendance

  • You must attend scheduled appointments or provide appropriate cancellation notice

  • Accurate and complete information must be provided during booking and treatment

  • You are responsible for following treatment recommendations and home exercise programs

  • Cancellations and no-shows are subject to our Cancellation Policy

Professional Boundaries

  • Clients must maintain appropriate professional relationships with staff and practitioners

  • Inappropriate behavior, harassment, or aggressive conduct will not be tolerated

  • We reserve the right to discontinue services for clients who breach professional boundaries

4. Health Information and Medical Disclaimer

Treatment Information

  • All treatment recommendations are based on professional assessment and clinical judgment

  • Information provided during consultations is for educational purposes related to your condition

  • You should seek immediate medical attention for any concerning symptoms or emergencies

  • Second opinions from other healthcare providers are always encouraged

Limitation of Medical Advice

  • Services provided do not replace comprehensive medical evaluation when indicated

  • Practitioners may refer you to other healthcare providers when appropriate

  • Treatment outcomes cannot be guaranteed and may vary between individuals

5. Privacy and Confidentiality

  • Communication with other healthcare providers involved in your care (with consent)

6. Payment Terms and Financial Responsibilities

Service Fees

  • Payment is due at the time of service unless alternative arrangements are made

  • Fees are subject to our Payment Terms policy and may change with notice

  • You are responsible for understanding your insurance coverage and any gap payments

  • Outstanding accounts may result in suspension of services

Insurance and Third-Party Payments

  • We accept various insurance providers and government schemes where applicable

  • Insurance claims are submitted as a service, but payment remains your responsibility

  • Pre-authorizations and approvals are your responsibility where required

7. Cancellation and Rescheduling

All appointment cancellations and rescheduling are subject to our Cancellation Policy:

  • Cancellation with notice (before midday, day before): No fee

  • Short notice cancellation (after midday, day before): $80 fee (may be waived if slot filled)

  • Late cancellation or non-attendance: Full session fee applies

8. Website and Online Systems

Online Booking

  • Online bookings are subject to confirmation by our reception team

  • You are responsible for maintaining confidentiality of any login credentials

  • Technical failures or system unavailability may occasionally occur

9. Limitation of Liability

To the fullest extent permitted by law, Lockyer Valley Allied Health's liability is limited to:

  • Business insurance covers premises-related incidents

10. Complaints and Dispute Resolution

Internal Resolution

  • Speak with your practitioner or reception team first

  • Management will investigate all formal complaints promptly

External Resolution

  • Formal complaints may be made to AHPRA or relevant professional bodies

  • Consumer complaints may be directed to Queensland Office of Fair Trading

  • Legal disputes are subject to Queensland jurisdiction

11. Termination of Services

We reserve the right to discontinue services if:

  • Payment obligations are not met

  • Professional boundaries are breached

  • Treatment goals are not being achieved

  • Mutual agreement that services are no longer appropriate

Reasonable notice will be provided where possible, and appropriate referrals offered.

12. Modifications to Terms

These Terms of Service may be updated periodically. Changes will be:

  • Posted in our clinic and on our website

  • Communicated to active clients where material changes affect services

  • Effective from the date specified in the updated terms

13. Governing Law

These Terms of Service are governed by Queensland and Commonwealth Australian law. Any disputes will be subject to the jurisdiction of Queensland courts.

14. Entire Agreement

These Terms of Service, together with our Privacy Policy, Cancellation Policy, Payment Terms, and Clinical Policies, constitute the complete agreement for services provided by Lockyer Valley Allied Health.
